Step-by-Step Procedures
Step 1: Secure NBI Fingerprint card (NBI Form No.5) at the Consulate General. The NBI Form No.5 is available for free at the Philippine Consulate General in Dubai. Ask the staff for assistance if needed.
Step 2: Fill out the personal data on both sides of the card legibly and truthfully. Use a black ballpoint pen and ensure all details are accurate, including your full name, civil status, date of birth, address, and gender.
Step 3: Request assistance/endorsement from the Consulate General or Police Agency to have your fingerprints impressed (Rolled Impression) on the appropriate spaces. Visit the Criminal Investigation Department (CID) at Al Mulla Plaza for fingerprinting. The person who takes your fingerprints must sign and state their official designation on the card.
Step 4: For married women, the order of writing is as follows:
- Maiden surname (family or surname of your father)
- First name or given name
- Maternal surname (maiden surname of your mother)
- Husband’s surname or family name (space provided, No.2)
Step 5: Attach the latest copy of your 2” x 2” photograph (white background), taken within three months before this application, to the appropriate box in NBI Form No. 5.
Step 6: Prepare two sets of documents, including attachments and a copy of the passport.
Step 7: Proceed to the Notary Window for processing.
Step 8: Pay the appropriate fee to the cashier (must be paid in Dirhams only). The standard fee is AED 100.00 per document, plus any applicable Courier Fees.
Step 9: Processing Schedule is 4 working days. After payment, please keep your receipt as it indicates when you can claim your clearance.
